Please check the form carefully before sending it. Occasionally, we are unable to reach someone due to a minor typo.

Step 3: Add the photos to the form

This form allows you to attach three photos. Add the three photos and then click 'Send'. We will send you a copy of the details you entered.

Step 4: We process your request

As soon as we receive your request, one of our doctors will review your photos. If the photos meet the requirements and the doctor can make a proper assessment, we will inform you of the findings by telephone. If the doctor is in doubt, for example because the photos are unclear, we prefer to invite you to the clinic.
